- Candidates with an AZ driver's license will not be considered - [REQUIRED]
- Provider must have 24 months minimum of current Progressive Care Unit/PCU experience - [REQUIRED]
- Certifications must be American Heart Association or American Red Cross - [REQUIRED]
- Nursys cannot show any hits on license at time of submission - [REQUIRED] [DOCUMENT]
- Banner will not approve RTO within the first week or holidays. Can not exceed 7 days - [REQUIRED]
- Flu vaccination - Banner declinations can be applied for (reach out for forms) **Please add vaccination status in the submission notes**
