Shift: MondayFriday days with potential weekend rotation and on call

Job Description:
Sunbelt Staffing is hiring a Hospice Home Health RN for a 13-week travel position in Mesa, AZ. This assignment provides the opportunity to deliver patient-centered hospice care while helping families navigate the challenges of end-of-life support. Candidates must have prior hospice experience and be proficient in Homecare Homebase.

Key Responsibilities:

• Provide skilled nursing visits for hospice patients in the home setting

• Manage symptoms and support patient comfort

• Create and adjust individualized care plans in collaboration with the hospice team

• Educate families and caregivers on care techniques and resources

• Ensure accurate documentation within Homecare Homebase

Qualifications:

• Current Arizona RN license

• At least 2 years of nursing experience, including 1 year in hospice home health

• Proficiency with Homecare Homebase (required)

• Strong communication and clinical judgment

• Ability to work independently while managing patient caseload

• Valid drivers license, reliable transportation, and auto insurance

• Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
