Job Description
h/care is excited to announce a vacancy for an Experienced Nurse Practitioner to join our Phoenix-based Palliative Care program. This position is full-time and requires the Nurse Practitioner to provide mobile, patient-focused care directly to individuals in their homes. The role is crucial in our mission to deliver superior medical services to patients with serious illnesses, where the focus is on optimizing comfort and quality of life. This is not a remote position, and the ideal candidate will be conducting on-site patient visits within the Phoenix metropolitan area.
As a healthcare provider dedicated to compassionate, comprehensive palliative care, our new team member will work independently to manage complex clinical cases. Duties and Responsibilities
- Perform comprehensive assessments to diagnose acute and chronic conditions.
- Develop and follow up on treatment plans that include prescription of medications, ordering of tests, and initiation of referrals to appropriate healthcare professionals.
- Provide in-home visits to manage complex chronic illnesses while addressing both the medical and social aspects of patient care.
- Engage in proactive symptom management for serious illnesses to enhance quality of life.
- Lead advanced care planning and goals-of-care conversations with patients and their families.
- Regularly document patient encounters and treatment plans in the electronic medical record (EMR).
- Collaborate closely with physicians, hospice care, and home health teams to create cohesive and comprehensive care plans.
- Provide mentorship and guidance to other healthcare professionals and support staff on palliative care principles and practices.
- Maintain a strong commitment to patient safety and compliance with healthcare standards and regulations.
- Manage emergency situations effectively using advanced clinical skills and sound judgment.
- Participate in continuous education to update knowledge and skills pertinent to the role and advancements in field-based healthcare practices.
Requirements
- Licensed Nurse Practitioner in the state of Arizona with 3-5 years of clinical experience.
- Experience in palliative care, hospice, home health, geriatrics, or managing complex chronic diseases is highly preferred.
- Active DEA license is required.
- Current BLS certification.
- Eligible to bill Medicare and other payors.
- Strong skills in symptom management, advance care planning, and goals-of-care conversations.
- Ability to work independently conducting in-home patient visits.
- Accurate and efficient documentation skills with EMR systems.
- Effective interpersonal and collaboration skills to work with various physicians and interdisciplinary care teams.
- Reliable transportation is necessary for field-based practice across the Phoenix metro area.
- Comfortable and capable of performing field-based practice in various home or community-based settings.
