The position is located in Sells, AZ.
Position Summary:
Under general supervision, provides educational programs and presentations, assists in the coordination and implementation of targeted outreach efforts to identify individuals who may be at risk for various mental and physical health conditions. Makes referrals to appropriate community service programs, may make home visits to monitor clients care and condition.
The work is normally reviewed upon completion, with occasional spot checks while in process, to ensure accuracy, timeliness and conformance to established standards; major work tasks are covered by instructions or procedures and unusual situations are referred to a supervisor.
Essential Duties and Responsibilities (Depending on area of assignment, incumbent may not be required to perform some of the duties listed below) :
- Assists with developing and implementing specific prevention techniques and programs.
- Identifies support services to be provided for community members.
- Facilitates prevention program meetings and activities and facilitates support meetings.
- Provides referral services for community members as identified.
- Provides education and information to the various Tohono O'odham Communities.
- Increases awareness of prevention techniques.
- Assists with presentations, workshops, in-services and health fairs.
- Organizes education materials and conducts presentations.
- Develops flyers, brochures and other informational materials.
- Transports and escorts clients to clinics, meetings or other activities as necessary.
- Maintains accurate contacts and logs of service delivery for database development.
- Attends meetings with outside agencies in order to coordinate efforts and keep updated on new information and treatment options.
- Performs other job related duties as assigned and contributes to a team effort.
Knowledge, Skills, and Abilities:
- Knowledge of the Tohono O'odham culture, customs, and traditions.
- Knowledge of the economic, educational, health and social problems of Native Americans.
- Knowledge of specific area of assignment, i.e. HIV/AIDS, Suicide Prevention and Drug and Alcohol Prevention and Recovery.
- Knowledge of appropriate resources and referral agencies.
- Knowledge of applicable federal, state, tribal laws, regulations and requirements.
- Skill in operating various word-processing, spreadsheets, and database software programs.
- Skill in interviewing and communicating to illicit information and gain cooperation from clients.
- Skill in coordinating and planning programs and other educational and outreach activities.
- Skill in providing superior customer service to external and internal customers.
- Ability to handle confidential information with proper discretion.
- Ability to present educational programs in front of large and small groups and create educational materials.
- Ability to communicate efficiently and effectively both verbally and in writing.
- Ability to establish and maintain positive and effective working relationships with other employees and the general public.
- Ability to maintain privileged confidential information.
- Ability to work independently and meet strict time lines.
- Ability to work extended hours and various work schedules including nights, weekends and holidays.
- Ability to travel long distances throughout the Tohono O'odham Nation.
Minimum Qualifications:
Associates degree in Social Work or closely related field and three years' work experience in outreach, prevention, recovery/support field, or an equivalent combination of training, education, and work experience, which demonstrates the ability to perform the duties of this position.
Licenses, Certifications, Special Requirements:
- Depending on area of assignment, may be required to obtain a Chemical Dependency Counseling Certification or licensure as a Substance Abuse Counselor.
- Upon recommendation for hire, a criminal background and National FBI check is required to determine suitability for hire, including a 39-month Motor Vehicle Record.
- May require possessing and maintaining a valid driver's license, (no DUIs or major traffic citations within the last three years).
- If required, must meet the Tohono O'odham Nation tribal employer's insurance requirements to receive a driver's permit to operate program vehicles.
- Based on the department's needs, incumbents may be required to demonstrate fluency in both the Tohono O'odham Language and English as a condition of employment.
