Banner Submission Requirements AZ home address on driver’s license not accepted (copy required with submission). Must use Banner-specific cover page – 100% accurate and complete. List all locations, shifts, and departments the traveler is willing to accept. Include all states of interest (e.g., Any AZ, Any CO, Any State). Start date must match or be very close to the order start date. RTO: Must be listed on cover page Less than 7 days total Not allowed during first week or holidays Candidate must be pre-closed and ready to accept. Notify immediately if traveler withdraws or becomes unavailable.
