Your Role
Provide skilled nursing care and manage symptoms in the patient’s home
Develop individualized care plans with the interdisciplinary team
Communicate and educate patients and families in Spanish and English
Document care accurately and efficiently
Participate in shared on-call responsibilities
What You Bring
Current Arizona RN license
Fluency in Spanish and English
At least 1 year of nursing experience (hospice/palliative care preferred)
Associate’s or Bachelor’s degree in Nursing
CPR certification, valid driver’s license, reliable vehicle, and insurance
Compassion, strong clinical judgment, and ability to work independently
