Your Role

• Provide skilled nursing care and manage symptoms in the patient’s home

• Develop individualized care plans with the interdisciplinary team

• Communicate and educate patients and families in Spanish and English

• Document care accurately and efficiently

• Participate in shared on-call responsibilities

What You Bring

• Current Arizona RN license

• Fluency in Spanish and English

• At least 1 year of nursing experience (hospice/palliative care preferred)

• Associate’s or Bachelor’s degree in Nursing

• CPR certification, valid driver’s license, reliable vehicle, and insurance

• Compassion, strong clinical judgment, and ability to work independently
