• Assess and evaluate patients' therapeutic, rehabilitative, and functional status to develop a personalized care plan.

• Evaluate home environments to ensure optimal recovery conditions.

• Assess muscle strength, mobility, gait, and range of motion (ROM) .

• Implement and oversee physical therapy treatment plans based on physician recommendations.

• Educate and guide patients, caregivers, and family members on therapy techniques and goals.

• Identify additional home health services needed and make appropriate referrals.

• Document and track patient progress through clinical reports.

• Provide in-service education for nursing staff as needed.

• Participate in peer consultations for continuous professional growth.

• Supervise Physical Therapy Assistants per company policy and state regulations.

• Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.

• Active state licensure as a Physical Therapist.

• Minimum of two years of experience as a Physical Therapist ( home health experience preferred ).

• Strong verbal and written communication skills and organizational abilities .

• Current CPR certification .
