COTA - Travel Contract
We‘re hiring a Certified Occupational Therapy Assistant for a travel contract role at a skilled nursing facility in Lakeside, AZ. Under the guidance of a licensed Occupational Therapist, you will help patients regain the skills they need for everyday life. Minimum Requirements
- Associate degree in Occupational Therapy from an accredited college.
- Active Arizona COTA license or ability to obtain required state licensure prior to start.
- BLS Certification from the American Heart Association or American Red Cross may be required.
- Experience working in a skilled nursing facility or long-term care environment preferred.
- Ability to work within a team under the direction of a licensed Occupational Therapist.
- Client requirement: availability for a 32-hour weekly schedule, 5 days per week (per client notes).
- Preferred: prior experience with geriatric populations, ADL retraining, mobility aids, and restorative nursing programs.
Assignment Details
- Facility Type: Skilled Nursing Facility.
- Shift Name: Days.
- Schedule: 4x8 shifts (per client notes).
- Assignment Length: 13-week travel contract.
- Floating: Candidate may be required to float to other facilities as needed.
Location Highlights
Experience Lakeside, AZ with access to outdoor recreation and small-town amenities. The area is near the Mogollon Rim and within about a 1–1.5 hour drive to Payson and its hiking areas, offering forested trails and fishing opportunities. The community is adjacent to several lakes including local reservoirs and is approximately a 2-hour drive from Flagstaff, providing access to higher-elevation trails, skiing in season, and historic downtown attractions. The setting fits a suburban-to-rural lifestyle with easy access to outdoor adventure, local dining, and community events, while being within reasonable driving distance to larger regional services and transit corridors.
