Salary: $85,000-$95,000 per year

Haven Home Health is seeking a compassionate and skilled Psychiatric RN to join our dedicated home health team serving patients throughout the Scottsdale area. Responsibilities

• Conduct comprehensive assessments of patients psychiatric and overall health needs
• Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary care team
• Administer medications and monitor patients for effectiveness and potential side effects
• Provide medication management and education to patients and families
• Provide skilled nursing care when needed, including treatments such as wound care, medication management, and other medical interventions in accordance with the patients plan of care
• Provide education and support to patients and families regarding psychiatric conditions, medications, and treatment plans
• Monitor patient progress and document findings in compliance with home health and regulatory standards
• Communicate effectively with the interdisciplinary team to ensure coordinated, high-quality patient care
• Advocate for patients while promoting mental health wellness and recovery in the home setting

Qualifications

• Active Registered Nurse (RN) license in Arizona
• 12 years of psychiatric nursing experience preferred
• Home Health experience is a plus
• Ability to provide both psychiatric care and skilled nursing treatments in the home setting
• Strong clinical assessment skills and ability to work independently in the field
• Excellent communication and patient-centered care approach
