We are seeking a Registered Nurse (RN) Case Manager to join our team. The ideal candidate is organized, compassionate, and able to communicate effectively with patients, providers, and staff to ensure coordinated and high-quality care.
Salary: Based on experience
Responsibilities:
Coordinate patient care and follow treatment plans
Communicate with patients, providers, and community resources
Assist patients in accessing healthcare services and support programs
Monitor patient progress and provide follow-up as needed
Maintain accurate documentation in the EHR system
Requirements:
Active RN license
Strong communication and organizational skills
Ability to work collaboratively with a healthcare team
Case management or behavioral health experience preferred
Marcann Mental Health Services is a small business in Arizona. Our goal is to provides the highest quality mental health services to adults in the Phoenix Metro and surrounding area. We utilize psychotherapy, medications, and alternative therapies to improve overall mental health and function in people with mental disorders. We take our care to the patient, visiting them in Skilled Nursing Facilities, Assisted Living, Memory Care Units, Group Homes and our office setting in Phoenix and Sun City West.
