• Assess and evaluate patients' therapeutic, rehabilitative, and functional needs.
• Develop and implement individualized care plans tailored to each patient’s condition.
• Evaluate home environments for safety and recommend modifications as needed.
• Provide direct physical therapy treatments and educate family members or caregivers.
• Identify additional home health services needed and make appropriate referrals.
• Prepare and submit clinical progress reports based on goal achievement.
• Offer in-service education for nursing staff as necessary.
• Supervise Physical Therapy Assistants in accordance with policies and state regulations.

• Education: Bachelor's degree in Physical Therapy or higher from an accredited institution.
• Licensure: Active Physical Therapist license in Arizona.
• Experience: At least one year of relevant experience; community/home health experience preferred.
• Language Skills: Bilingual (English/Spanish) strongly preferred.
• Skills: Strong verbal and written communication skills with excellent organizational abilities.
• Certifications: Current CPR certification.
