Responsibilities

• Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts.
• Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals.
• Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary to ensure proper nutrition.
• Administer medications following prescribed care plans while maintaining compliance with HIPAA regulations to protect patient privacy.
• Support patients during outings or community activities to promote social engagement and independence.
• Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces.
• Document care activities meticulously in accordance with agency protocols and update care plans regularly to reflect evolving needs.

Qualifications

• Prior experience in home health care, assisted living, nursing homes, or group homes is highly preferred.
• Ability to pass a background check is required.
• Certification in First Aid and CPR is required; additional training in Alzheimer's care, dementia care, hospice care or working with individuals with developmental disabilities is a plus.
• Strong patient observation skills to identify changes in condition or behavior promptly.
• Ability to lift heavy objects safely and assist with transfers using equipment like Hoyer lifts or resident’s .
• Knowledge of medication administration procedures and vital signs monitoring.
• Excellent communication skills for collaborating with clients’ families and healthcare teams while maintaining confidentiality under HIPAA guidelines.
