Position Summary:
The Clinical Social Worker supports the delivery of health care by providing social work services to identified beneficiaries. Under limited supervision, plans and implements services and resources that include approved activities that best meet the clients' needs, based on the program's goals and objectives.
The work is generally reviewed upon completion to ensure compliance with general organization/ department goals, policies, and procedures. Procedures or guidelines cover major work activities permitting flexibility in selecting methods to utilize, processes to follow, determining work priorities, and planning and coordinating projects.
Scope of Work:
The position is located within the Tohono O'odham Nation Sells Hospital, San Xavier Health Center, and San Simon Health Center. The incumbents assist with alleviating the social and emotional impact of physical and mental illness and disability. Supervisor and incumbent outline or discuss possible problem areas and define program objectives, plans, priorities, and deadlines. The social worker makes independent professional decisions and recommendations.
Essential Duties and Responsibilities: (Depending on the area of assignment, an incumbent may not be required to perform some of the duties listed below):
- Provides clinical and medical social work services to identified individuals and families that require behavioral, medical social work support and referral to other resources.
- Provide assessment, diagnosis, and treatment of mental health disorders.
- Performs initial screening to determine appropriateness for services.
- Provides biopsychosocial needs assessment in the hospital inpatient setting in the context of multi-disciplinary discharge planning.
- Provides crisis intervention, stabilization, and referral as needed in outpatient and emergency room settings.
- Serves as a client advocate referral and information resource, referring hospital and clinic beneficiaries to other agencies and resources as needed.
- Documents timely and accurately the results of the assessment, treatment, and progress notes; generates reports as required to quantify clinical activities.
- Evaluates individual client progress to ensure ongoing improvement and participation in care.
- Develops and maintains ongoing working relationships with representatives from tribal, local, state, health, and welfare agencies and organizations.
- Maintains communication with other resources and agencies, following up on all referrals to ensure appropriate services are provided.
- Assists in planning, designing, and implementing program improvement processes.
- Maintains organizational awareness of program activities by participating in staff meetings; attending scheduled supervisory meetings.
- Guide the hospital organization that includes the safety and security of beneficiaries, employees, and property in the absence of the Supervisor.
- Maintains professional licensure, knowledge, and skill sets through continuing education; by attending seminars, workshops, and conferences; by reviewing professional publications and membership in professional organizations.
- Maintain complete confidentiality of all administrative, patient medical records, and other pertinent information in compliance with the Privacy Act of 1974 and all provisions of the HIPAA privacy rule.
- Performs other job-related duties as assigned.
- Contributes to the organization's team effort to integrate medical and behavioral health care.
Knowledge, Skills, and Abilities:
- Knowledge of Tohono O'odham traditions, language, history, and culture.
- Knowledge of the economic, educational, and health, and social problems of Native American children, adults, elders.
- Knowledge and application of social work theory, knowledge, methods, and ethics to enhance the biopsychosocial functioning of individuals, couples, families, groups, organizations, and communities.
- Application of specialized clinical knowledge and advanced clinical skills in assessment, diagnosis, and treatment of mental, emotional, and behavioral disorders, conditions, and addictions.
- Knowledge of human development, theories of behavior, clinical theory, and intervention models.
- Knowledge of theories of personality, psychotherapy, and counseling.
- Knowledge of family theory - including family systems, diverse family structures, family development over time.
- Knowledge of biopsychosocial assessment.
- Knowledge of diversity challenges of clinical practice - race, ethnicity, sexual orientation, spirituality/religion, variations in ability/ disability.
- Knowledge of applicable federal, state, tribal laws, regulations, and requirements.
- Knowledge of the organization's mission, organization, programs, and requirements of a health care delivery system.
- Knowledge of social policies, services, and resources, especially in clinical practice.
- Knowledge in records management procedures and documentation and use of electronic health records system.
- Skills in assessing, interviewing including sensitivity and competence with diverse populations.
- Skills in case advocacy resource development, management.
- Skill in operating various word-processing, spreadsheets, and database software programs.
- Skill in analyzing problems, projecting outcomes, identifying solutions, and implementing recommendations.
- Skill in providing superior customer service to external and internal customers.
- Ability to exercise independent judgment.
- Ability to communicate efficiently and effectively both verbally and in writing.
- Ability to work effectively with individuals and demonstrate leadership and team-building skills with empathy and enthusiasm.
- Ability to handle multiple tasks and meet deadlines.
- Ability to work extended hours and various work schedules.
- Ability to establish and maintain positive and effective working relationships with other employees and the general public.
- Ability to travel within the interior/exterior boundaries of the Tohono O'odham Nation.
Minimum Qualifications:
- Master's Degree in Social Work (MSW) from an institution accredited by the Council on Social Work Education; and two years of supervised training and experience after graduation; to include certified evidence-based treatment framework with adults, children, elders, and group therapy in a clinical setting, or equivalent.
Licenses, Certifications, Special Requirements:
- Must possess a valid and current LCSW - Licensed Clinical Social Worker.
- Upon recommendation for hire, a criminal background and a National FBI fingerprint check are required to determine suitability for employment, including a 39-month driving record.
- May require possessing and maintaining a valid driver's license (no DUIs or major traffic citations within the last three years).
- If required, must meet the Tohono O'odham Nation tribal employer's insurance requirements to receive a driver's permit to operate program vehicles.
- Based on the department's needs, incumbents may be required to demonstrate fluency in both the Tohono O'odham language and English as employment conditions.
