Occupational/Home Health Therapist $1500 Sign-On Bonus
Job Position: Home Health Occupational Therapist

Qualifications:
- Current license as an OT in AZ.
- Home Health experience.
- Compliance with professional standards and practice.
- Excellent verbal and written communication skills and strong interpersonal skills.
- Has knowledge of home care federal and state regulations.
- Licensed driver with automobile insured in accordance with the Agency and state requirements.
- Has current CPR card.
- Has current TB test
Responsibilities:
- Follow an individual Plan of Care
- Provide skilled interventions with a focus on achieving realistic client outcomes within a specified time period.
- Report significant changes in client status to RN, physician, and other members of the team in a timely manner consistent with client needs.
- Consistently demonstrate competency with therapy skills according to the personal and legal scope of practice.
- Practice therapy with respect for individual, cultural, and spiritual differences.
- Maintain client records showing intervention and education.
- Ensure accuracy, completeness, and timeliness of clinical documentation in accordance with Agency policies and procedures, regulatory requirements, and industry standards.
- Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures and regulatory requirements.
- Promote personal safety and a safe environment for clients and co-workers.
ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
1. Performs occupational therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluation of patients who are receiving services under a medically approved Plan of Care.
2. Completes assessments in a timely manner and in accordance with Agency policy.
3. Develops a treatment program and establishes goals for improved function.
4. Provides assessment findings and goals to assist in developing the patient care plan and obtains specific physician orders as needed.
5. Documents all findings, plans, interventions, and patient progress (outcomes) on designated forms and submits per Agency guidelines.
6. Communicates plans and changes to the physician and to nursing Case Manager and other agency staff through the care plan, written progress notes, and participation in care conferences.
7. Guides and instructs the patient in prescribing therapeutic self-care and creative activities that are directed toward improving independence and physical and cognitive functioning.
8. Advises, consults, and teaches appropriate treatment procedures and supportive activities to patients, family members, and other Agency staff.
9. Designs and/or orders appropriate adaptive/assistive devices and/or splints to promote optimal level of functioning.
10. Instructs patients and their families in the patient specific program.
11. Performs job in compliance with Agency policies and procedures and professional and community standards.
12. Provides services in accordance with applicable state, federal, and JCAHO standards.
13. Attends meetings and required educational programs.
14. Accepts responsibility for personal and professional development and identifies learning/developmental needs to supervisor.
15. Attends care conferences, initiates interdisciplinary referrals and participates in care coordination as needed.
16. Serves on the Clinical Record Review Committee.
17. Performs other related duties and responsibilities as deemed necessary.
SUPERVISION OF OCCUPATIONAL THERAPY ASSISTANTS
1. Instructs and supervises Occupational Therapy Assistants in certain phases of occupational therapy services they may provide to the patient.

18. Supervises on-site while the Occupational Therapy Assistant is rendering patient care—every four (4) to six (6) visits, minimally once every month. Supervision does not constitute treatment.
19. Completes an on-site functional assessment, review, and revision of the Plan of Care, and an assessment of utilization of community resources.
