  • Follow care plans and communicate any changes in health or behavior to the care team.
• Transportation: Keep clients connected to their community by assisting with errands, appointments, and social activities.
• Household Organization: Maintain a clean, safe, and welcoming home environment through light housekeeping and laundry.
