Registered Nurse - Clinical Care Guide/Case Manager
We are seeking a compassionate, highly organized Registered Nurse Clinical Care Guide to join our Advanced Primary Care team in Scottsdale, AZ. In this patient-centered role, you will provide end-to-end support for specialist and diagnostic testing navigation, ensuring patients receive seamless, coordinated care throughout their health journey. As a key clinical partner, you will collaborate closely with providers, care coordinators, and support staff to help patients achieve their best possible health outcomes.

Key Responsibilities:
Serve as the main point of contact for specialist and diagnostic testing navigation, including scheduling and follow-up.
Support patients through individualized care plans and promote proactive health management.
Conduct clinical assessments and provide guidance aligned with the Advanced Primary Care model.
Assist with care transitions, including post-discharge follow-up and coordination with community resources.
Maintain accurate electronic medical documentation.
Provide excellent customer service and build strong patient relationships.
Perform other duties as assigned.
Work flexible hours, including mornings, evenings, or weekends.
Travel between care centers as needed; position is 100% on-site.
Requirements:
Current Arizona Registered Nurse license.
BLS certification required.
Experience with Electronic Medical Records (EMR), including Medicare-based systems.
Strong customer service orientation with experience in patient-facing roles.
Excellent verbal, written, and electronic communication skills.
Active AZ Fingerprint Clearance Card required.
