As a Full-Time Medical Social Worker at American Premier Hospice, your daily expectations will include conducting comprehensive assessments of patients’ social, emotional, and environmental needs. You will develop and implement individualized care plans, collaborating closely with healthcare professionals and family members to ensure holistic support. You will facilitate access to community resources, helping patients navigate healthcare systems and addressing any barriers to care.

Providing emotional support through counseling sessions will be a crucial part of your role, enabling patients and families to cope with illness and transitions. Regular documentation of care activities and progress notes will be required to maintain accurate records for compliance and care coordination. Additionally, you’ll participate in team meetings and case discussions, bringing your insights to enhance patient care strategies.

• To excel as a Full-Time Medical Social Worker at American Premier Hospice, candidates must possess a blend of education, experience, and essential skills.
• A Bachelor’s or Master’s degree in Social Work is required, with a preference for those holding a Licensed Clinical Social Worker (LCSW) designation. A minimum of 2 years of hospice or palliative care experience is necessary, emphasizing expertise in psychosocial assessments, grief counseling, and family dynamics. Successful applicants will demonstrate strong emotional support and counseling skills, enabling them to provide effective grief counseling and crisis intervention.
• Knowledge of end-of-life care principles and a compassionate approach to advance care planning are essential.
• Exceptional communication skills are critical for facilitating difficult discussions regarding end-of-life options. Proficiency in advocacy and resource navigation empowers you to connect families with necessary community services. Additionally, experience in interdisciplinary team collaboration, innovative therapy techniques (like CBT and EMDR), and tech-savviness with electronic medical records will further contribute to your success in this role.
• Lastly, an understanding of regulatory compliance is important to ensure adherence to Medicare and Medicaid regulations and ethical guidelines in social work practice.
