Position Overview

We are seeking an experienced Nurse Practitioner with advanced expertise in Functional Medicine to join our team at Relive Health Tempe. The ideal candidate is passionate about hormone optimization, peptide therapy, and medical weight loss - and thrives on building meaningful, long-term client relationships.

Responsibilities

Perform comprehensive exams, review labs, and conduct advanced functional health assessments

Develop personalized treatment plans (HRT, peptide therapy, IV therapy, supplements, lifestyle coaching)

Translate complex medical insights into clear, actionable guidance for clients

Collaborate with a multidisciplinary team of providers, nurses, and client advocates

Maintain detailed documentation in the EHR while ensuring compliance with all regulations

Qualifications

Active NP license in Arizona

5+ years' clinical experience with a focus on functional medicine, hormone optimization, peptide therapy, and medical weight loss

Demonstrated expertise in root-cause medicine and evidence-based wellness approaches

Excellent communication and client-relationship skills

Strong commitment to proactive, patient-centered care
