This includes, but is not limited to: Phoenix, Scottsdale, Paradise Valley, Gilbert, Mesa, Chandler, Cave Creek, Carefree, and Ahwatukee. Description: The Caregiver/CNA is responsible for providing high quality, safe and supportive care to patients in the comfort of their own homes. In addition, the Caregiver/CNA will assist the patients with ADLs (Activities of Daily Living), and other necessary task that are essential to their every day living. Job Duties and Responsibilities:

• Personal Care- Assist with personal hygiene, bathing, showering, and dressing.
• Mobility- Assist with ambulation, physical therapy exercise and walking inside and outside the home.
• Meals- Assist with the planning and preparation of meals
• Housekeeping- perform light housekeeping duties where needed (dishes, bedding, laundry, dust, sweep, mop, etc.).
• Reminders- Remind clients to take prescription medications and of scheduled appointments.
• Errands- Complete shopping task or accompany patients while they shop.
• Transportation- Provide safe and reliable transportation to appointments, shopping, pharmacy, and other errands as needed.

Education, Certification, License:

• High School diploma or GED equivalency.
• Current TB Testing
• CPR/First Aid (will provide resources if needed).
• Fingerprint Clearance Card or Background Check
• Minimum of 2-3 years of experience in healthcare, hospitality, or in-home care environment.
• Hoyer Lift Experience (client specific requirement).
• Ability to lift up to 25lbs
• Valid Driver’s License and reliable transportation required.
• Must be 18yrs or older
