Whether you are an experienced home health clinician or looking to transition into home health, we would love to speak with you.

What You’ll Do:

• Perform speech therapy evaluations and reassessments in patients’ homes.
• Develop individualized treatment plans based on physician orders.
• Treat speech, language, cognitive, communication, and swallowing disorders.
• Educate patients and caregivers on therapy techniques and safety strategies.
• Collaborate with nurses, therapists, physicians, and interdisciplinary team members.
• Complete accurate and timely documentation in compliance with Medicare and Arizona regulations.
• Participate in case conferences as needed.

Qualifications:

• Current Arizona Speech Language Pathologist license in good standing.
• Master’s degree in Speech Language Pathology.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Strong communication and documentation skills.
• Home health experience preferred, but not required.
