Primary Care Physician - Phoenix, AZ

Compensation : $270,000 BASE, with a total potential of $324,000 + Full Benefits

We are a new, innovative healthcare provider dedicated to improving our patients' lives. We deliver top-quality care at accessible neighborhood clinics where patients feel at home and become part of a wellness-focused community. Our patients benefit from greater continuity of care and the reassurance of being treated with respect by caring professionals. The center offers Uber/Lyft transportation for those who don't drive, as well as free activities like exercise and art to promote better health for seniors.

Role:

• Compensation: Base $270,000 + up to 20% of base salary in bonuses
• Bonus Criteria: Patient satisfaction, living the companys values, HEDIS compliance, reducing hospital sick days
• Position: Primary Care Outpatient
• Start Date: May 2025 (new center opening)
• Care Model: Value-Based Care
• Patient Population: Seniors with 3-5 complex diseases (e.g., heart disease, diabetes, hypertension)
• Average Patients per Day: 10-12
• Panel Size: Max 650 patients
• Shifts: Monday-Friday, 8 am - 5 pm
• Call Schedule: Based on provider numbers in the market (e.g., 1:32 weeks if 32 physicians)
• Support: Care teams with MAs, schedulers, referral specialists, and leadership support

Requirements:

• MD or DO
• Board Certified or Eligible in Family Medicine or Internal Medicine
• Fellowship in Geriatrics is a plus but not required
• Active, unrestricted medical license and DEA registration
• Proficient in office procedures: suturing, joint injections, skin biopsies, basic wound care
