What you'll do:

As a Bilingual Advanced Practitioner, you'll be a vital part of an integrated care team, working alongside a Registered Nurse and Social Worker. You'll provide personalized, compassionate medical care to patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD), empowering them to improve their quality of life from the comfort of their homes and slowing the progression of their condition. Here's a glimpse of what you'll be doing:

Conducting comprehensive assessments, including annual wellness exams, both in patients' homes and via telehealth.
Counseling and educating patients and families about available benefits and programs, ensuring they have the resources they need.
Thoroughly documenting patient encounters, including chief complaint, diagnoses, history, review of systems, examinations, medications, allergies, assessments, and plans.
Coordinating care with primary care providers, specialists, and ancillary services to ensure seamless collaboration.
Completing all necessary documentation accurately and promptly.
Maintaining the highest standards of care.
Delivering evidence-based, timely care that reduces avoidable hospitalizations and prioritizes patient well-being and satisfaction.
Prescribing medications, ordering tests, and collaborating with the patient's physician.
Working to improve patient quality outcomes through education and preventive monitoring.
Assisting patients in accessing educational videos and resources.
Actively participating in integrated care team meetings.

What you'll bring to the table:

Fluency in English and Spanish is required.
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work independently and autonomously.
Access to reliable transportation, a valid driver's license, and car insurance.
Proficiency with medical instruments and equipment.
Knowledge of computer-based data management programs and information systems.
Excellent verbal and written communication skills.
A strong understanding of federal and state regulations (HIPAA and OSHA).
Two or more years of direct patient care experience.
Experience with managed care/IPA/health plans.
Experience conducting annual wellness visits or similar comprehensive visits (virtually or in-home).
