Home Health Intake & Eligibility Coordinator (Medical Assistant Preferred) Location: Chandler, AZ

Bridges Health Services is growing across California and Arizona. We’re hiring a detail-oriented Intake & Eligibility Coordinator to manage incoming referrals, run daily insurance eligibility, and complete high-touch patient outreach. This role partners closely with referral sources, patients/families, and our clinical scheduling team to ensure a fast, compliant start of care.
What you’ll doReferral intake & onboarding
Receive and process referrals from hospitals, SNFs, clinics, and community partners.
Create/complete intake records; collect demographics, consent, physician orders, and required documents (face-to-face, plan of care, etc.).
Enter and maintain accurate data in the EMR; open episodes and route tasks to clinical/scheduling teams.
Daily eligibility & authorizations
Run eligibility/benefits every day for Medicare, Medi-Cal, AHCCCS (AZ), and commercial payers using payer portals/clearinghouses.
Document coverage, copays/deductibles, and home health benefit criteria; identify auth requirements and obtain/track approvals.
Flag and escalate non-covered services or missing documents before scheduling.
Patient & referral communications
Make timely outreach calls (inbound/outbound) to patients/families to verify information, explain next steps, and schedule start-of-care windows.
Provide referral sources with prompt status updates and resolve issues professionally.
Compliance, quality & throughput
Safeguard PHI in line with HIPAA and company policy.
Meet or exceed service levels (e.g., referral-to-first-attempt within 1 business hour; same-day eligibility completion; error rate <2%).
Maintain clean dashboards/queues; close loops on all open tasks daily.
1–2+ years in home health, medical intake/admissions, front office, or revenue cycle (eligibility/benefits).
Medical Assistant (MA) training/certification preferred; CNA or medical office experience also considered.
Working knowledge of Medicare, Medi-Cal, AHCCCS, and major commercial plans ; prior-auth basics.
EMR proficiency (e.g., Axxess, WellSky, HCHB) and comfort with payer portals (e.g., Availity, UHC, BCBS).
Clear, warm phone etiquette; high volume call handling; strong writing/spelling in documentation.
Organized and deadline-driven; able to juggle multiple referrals and maintain accuracy.
Bilingual (English/Spanish) a plus.
Uses EMR(Data Soft Logic), Google Workspace (Docs/Sheets/Drive), secure fax/e-fax, and payer clearinghouses/portals.
Arizona postings: $18–$23 per hour DOE, plus benefits.
Comprehensive benefits package may include medical/dental/vision, paid time off, paid holidays.
Note: Final pay is based on location, experience, and internal equity. Bridges Health Services is an at-will employer.
