Foundation Home Health is seeking a dynamic, energetic, and empathetic Licensed Physical Therapist Assistant (PTA) to join our growing interdisciplinary team serving the Lake Havasu City area.

Key Responsibilities

The Home Health PTA provides therapy services as prescribed by the supervising Physical Therapist and physician, ensuring optimal outcomes for the patient.

Therapy Delivery & Execution

• Implement and progress the physical therapy treatment plan as established by the supervising Physical Therapist.
• Perform a variety of therapeutic interventions, including therapeutic exercises, gait training, transfer training , balance activities, and the use of physical modalities.
• Observe, document, and report the patient's response to treatment , including any changes in condition or tolerance, to the supervising PT promptly.
• Instruct patients and caregivers in the use of assistive devices (walkers, canes) and adaptive equipment to enhance safety and mobility.

Safety & Documentation

• Educate patients and families on proper techniques to ensure patient safety and maximize carry-over of exercises between visits.
• Conduct thorough home safety assessments related to mobility and function and recommend appropriate modifications or equipment.
• Maintain accurate, timely, and complete documentation of all treatments, interventions, and patient responses in the electronic health record (EHR).
• Adhere to all federal, state, and Foundation Home Health regulations regarding physical therapy assistant practice and billing compliance.

Team Collaboration

• Participate in case conferences and communicate effectively with the Physical Therapist regarding the patient's progress toward established goals.
• Coordinate scheduling and care with the LPN, RN, and other therapy disciplines involved in the patient’s plan of care.

✅ Qualifications

Required

• Current, valid, and unrestricted Physical Therapist Assistant (PTA) license in the state of Arizona.
• Graduation from an accredited Physical Therapist Assistant program.
• Current Basic Life Support (BLS) certification.
• A minimum of one (1) year of recent clinical experience as a PTA; home health experience is highly preferred.
• Proven ability to work independently, prioritize tasks, and manage a caseload effectively.
• Reliable transportation, a valid Arizona Driver's License, and current auto insurance, as daily travel is essential.

Preferred

• Familiarity with Medicare guidelines related to home health documentation and care.
• Strong knowledge of geriatric care and common orthopedic/neurological conditions.
