Shift: Monday to Friday 4:30am to 12:30pm

Community Medical Services (CMS) is hiring a Clinical Documentation Coordinator. Under the supervision of the Clinic Manager the Clinical Documentation Coordinator is responsible for managing the end-to-end flow of client documentation to ensure all required forms are completed signed uploaded and accurately linked to the correct client chart within established timeframes. This role works closely with Counselors Client Service Coordinators and Nursing staff on a daily basis to maintain timely and compliant records that support quality client care.

Responsibilities:

  • Performs general office duties such as document preparation scanning faxing
  • Ensures confidentiality of all client and employee information.
  • Collect required forms (in-person and remote).
  • Track missing items and rout to the right person (client/counselor/nurse) for completion.
  • Collect signatures on required documents.
  • Monitor signature status and escalate delays.
  • Scan documents and link them to the correct client chart.
  • Facilitate internal transfers to other CMS locations as well as transfers to external MAT programs.
  • Maintain and track forms for transfer process.
  • Assist with completion of required intake documents and forms collecting signatures and routing as appropriate.
  • Assist counselors with scanning/uploading/faxing forms for courtesy dosing and request exceptions for take-home medication policy.
  • Assist Client Service Coordinators with the patient flow of a high volume of clients in a fast-paced environment.
  • Maintains a welcoming lobby and reception area by ensuring areas remain clean and organized.
  • Recovery Connect engagement app and help them navigate it as needed.

Requirements

  • High school diploma or GED required
  • Bilingual in English/Spanish preferred
  • 1 years of experience in customer service required
  • 1 years of front desk experience required
  • Front desk experience in a Hospital or multi-physician practice preferred
  • Behavioral health work experience preferred

Skills:

  • Ability to communicate clearly and effectively between all organizational levels and with outside providers
  • Bilingual preferred but not required
  • Intermediate computer knowledge including ability to navigate in electronic health records
  • General knowledge and aptitude for working independently with adults of mixed ethnic groups
  • Culturally competent and sensitive to client and employee needs
  • Demonstrated ability in medication administration accuracy and ability to maintain accurate client records
  • Excellent organizational skills accuracy and attention to detail
  • Problem solving conflict resolution time management and strong customer service skills
  • Strong team player comfortable working in a fast-paced setting
  • Ability to maintain confidentiality to ensure compliance with HIPAA and 42 CFR Part 2
  • Flexible with the ability to work in a continuously changing environment

Tools and Equipment Requirements:

  • The ability to use a phone computer printer and copier is required.
  • Frequent use of Microsoft office products including but not limited to Outlook Word Excel Access Microsoft Teams and PowerPoint.
  • The ability to use the internet and various web browser software is required.

Physical Working Conditions and Office Setting Description:

  • Prolonged sitting standing frequent bending stooping or stretching associated with an office environment
  • Manual dexterity using a calculator and computer keyboard
  • Frequent and prolonged typing and operation of computer keyboard and telephones
