Job Summary
We are seeking a dedicated Quality Coordinator to join our growing market operations team (Glendale, Peoria, and Sun City). This role plays a key part in improving patient outcomes by driving CMS STARS and HEDIS quality initiatives in a senior-focused outpatient primary care setting. The ideal candidate will be proactive, detail-oriented, and passionate about providing high-quality care to our senior community.
Key ResponsibilitiesExecute both enterprise-level and local strategies to close STARS care gaps.
Review and abstract medical records to track and close Focus On Care measures.
Upload and manage documentation within the electronic medical record system.
Conduct patient outreach calls to schedule and coordinate quality-related screenings and appointments.
Schedule and conduct in-center ancillary visits for screenings and assessments.
Collaborate regularly with Market Leaders to monitor progress and plan improvement strategies.
Ensure compliance with OSHA standards and maintain accurate, legal, and ethical documentation.
Perform other duties as assigned to support quality initiatives.
Knowledge of quality measures and gap closure strategies (STARS/HEDIS).
Previous experience in a primary care or clinical setting.
Strong proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook, Teams).
Team-oriented with proven collaboration and communication skills.
High attention to detail and ability to work independently.
Cultural sensitivity and a patient-first mindset.
Commitment to maintaining HIPAA and confidentiality standards.
Minimum of 1 year working as a Medical Assistant.
CMS STARS/HEDIS program experience highly preferred.
Familiarity with EClinicalWorks or similar EMR platforms.
Experience working with geriatric populations is a plus.
High school diploma or GED required.
Graduation from an accredited Medical Assistant program or equivalent vocational healthcare training required.
BLS for Healthcare Providers certification required.
