Position Details:
• Status: Full-Time, Monday-Friday, 8:00 AM-5:00 PM
• Compensation: Full-Time RNs can earn $85K+ annually
• Work Area: East Mesa (Mesa, Apache Junction, Gilbert)

Your Role as RN Case Manager:

As a Home Health & Hospice Nurse Case Manager, you'll be instrumental in providing exceptional care to patients in their homes. Your role will include:
• Conducting comprehensive physical, social, and psychological assessments.
• Collaborating with patients, families, and the Interdisciplinary Group to create individualized care plans.
• Managing care delivery and maintaining meticulous patient records.
• Acting as a resource for families, making proactive referrals, and fostering a holistic care experience.

Qualifications We're Looking For:
• Active Registered Nurse (RN) license in Arizona.
• Previous Home Health or Hospice experience (preferred).
• Effective verbal, written, and time management skills.
• Valid driver's license, reliable transportation, and proof of current auto insurance.
• Ability to perform patient transfers with a full range of movement.
• Current CPR certification and negative TB test (we can assist).
• Valid AZ Fingerprint Clearance Card (we can help obtain it).
