Registered Nurse – Home Health needed for a travel assignment in Cottonwood, Arizona. Start date May 11, 2026 for 13 weeks with guaranteed 40 hours per week on an 8-hour shift. Responsibilities
- Assess and document patient needs in home health/hospice, including stable and emergent situations, to develop and update the Plan of Care.
- Deliver skilled nursing care in patients’ homes, ensure infection control, and educate patients/families on care, medications, safety, and self-care.
- Coordinate care with the interdisciplinary team, referring agencies, and family; supervise and provide clinical directions to HHA/STNA/LVN as appropriate.
- Monitor and reassess pain; implement pain management strategies and educate the patient/family on pain control.
- Maintain thorough, up-to-date patient records; participate in interdisciplinary team meetings and provide on-call coverage as assigned.

Requirements
- Current Arizona RN license
- CPR certification required
- OASIS experience required
- Homecare Homebase software experience required
- 2+ years home health experience; ADN or Nursing Diploma required; BSN preferred; CHPN preferred
- Valid driver's license and reliable transportation
