Job Title: Travel Licensed Practical Nurse (LPN) – Night Shift

Location: Flagstaff, AZ 86004

Job Type: Contract

Shift: 12-hour shifts, 36 hours/week (6:00 PM – 6:30 AM)

• High school diploma or equivalent

• Active LPN License (Arizona or Compact)

• Current CPR/BLS Certification

• Administer medications and treatments safely and promptly, in accordance with all applicable laws, regulations, and facility policies

• Document and communicate residents’ care and condition clearly and accurately, both verbally and in writing, to the interdisciplinary team and across care settings

• Perform resident assessments, prepare incident and infection control reports, and contribute to Quality Assurance and Performance Improvement (QAPI) initiatives

• Supervise nursing assistants during the assigned shift to ensure high-quality care and adherence to policy and procedure

• Assist in resolving workplace issues and participate in staff training as needed

• Advocate for residents’ rights, preferences, and care directives in the delivery of quality care

• Maintain professional conduct and confidentiality while interacting with residents, families, staff, visitors, and vendors

• Conduct ongoing care assessments, report findings to physicians as indicated, and initiate or update resident care plans

• Ensure accurate and timely documentation of care provided in residents’ medical records
