Position SummarySource of Hope LLC is seeking a Part-Time Customer Service Representative / Community Outreach Coordinator to serve as the first point of contact for prospective families and community partners. This role combines excellent customer service with community outreach and business development efforts.
The ideal candidate will be passionate about helping families navigate services, building relationships within the community, and promoting the mission of Source of Hope LLC. A key responsibility of this position is identifying, connecting with, and welcoming new families who may benefit from our services.
This position offers flexibility and the opportunity to make a meaningful impact by helping individuals and families access quality support services.
Essential Duties and ResponsibilitiesCustomer Service & Family Support- Answer incoming phone calls, emails, and inquiries from current and prospective clients.
- Provide accurate information regarding Source of Hope LLC services and programs.
- Assist families with understanding available services, eligibility requirements, and enrollment processes.
- Maintain professional, compassionate, and responsive communication with clients and families.
- Address questions, concerns, and service-related inquiries in a timely manner.
- Actively identify and engage prospective families who may benefit from Source of Hope LLC services.
- Monitor provider service hours to ensure authorized hours are utilized appropriately and efficiently.
- Track service utilization and identify underutilized or overutilized service authorizations.
- Communicate regularly with providers regarding available service hours, scheduling needs, and service delivery expectations.
- Build relationships with community organizations, schools, support coordinators, healthcare professionals, and advocacy groups.
- Attend community events, resource fairs, workshops, and networking opportunities to promote agency services.
- Conduct outreach efforts through phone calls, email campaigns, social media, and community engagement initiatives.
- Develop and maintain referral sources and community partnerships.
- Follow up with prospective families and guide them through the intake and enrollment process.
- Track outreach activities and maintain records of referrals and prospective clients.
- Maintain accurate records of client inquiries, referrals, and outreach activities.
- Assist with data entry, document collection, and client onboarding processes.
- Schedule appointments, meetings, and follow-up calls as needed.
- Support management with marketing and recruitment initiatives.
- Assist in maintaining client and provider databases.
- Represent Source of Hope LLC professionally within the community.
- Promote the organization's mission, values, and services to prospective families and community stakeholders.
- Build trust and establish long-term relationships with families and referral partners.
- Help increase community awareness of available HCBS and DDD services.
- High school diploma or equivalent required.
- Customer service, sales, outreach, marketing, healthcare, or social services experience preferred.
- Excellent communication and interpersonal skills.
- Strong organizational and time-management abilities.
- Ability to build rapport with individuals from diverse backgrounds.
- Comfortable speaking with families, community organizations, and referral sources.
- Proficiency in Microsoft Office, Outlook, and basic computer applications.
- Ability to work independently and meet outreach goals.
- Valid driver's license and reliable transportation for local travel.
- Experience working with individuals with developmental disabilities and their families.
- Knowledge of Arizona DDD services and HCBS programs.
- Experience in community outreach, recruitment, marketing, or relationship management.
