Job Responsibilities

1. Performs personal care activities contained in a written plan of care assignment which include: Bathing | Transfer | Fall Prevention | Light Housekeeping Dressing | Continence | Shopping/Errands | Medication Reminders Toileting | Feeding | Meal Preparation

2. Encouraging the patient to become as independent as possible according to the interdisciplinary care plan.

3. Attempting to promote patient's mental alertness through involvement in activities of interest.

4. Gives simple emotional and psychological support to the patient and other members of the household. Reports any change in the patient's mental or physical condition or home situation to the Nurse and members of the interdisciplinary team.

5. Detailed Charting on a carbon copy visit note

6. Practices infection control measures per OSHA standards.

Education And Experience:

• High School Diploma or GED
• Must be over 18 years old
• 0 - 1 years experience
• Must have a valid in-state driver's license, provide current registration and insurance, and have reliable transportation.

Compensation: $16.00 - $17.00 per hour
