We are a licensed hospice agency in the Phoenix area looking for a 1099 Hospice MSW Social Worker to provide quality, compassionate, compliant care for our patients who are now in their end of life stage.

The 1099 Hospice MSW Social Worker is a member of the Interdisciplinary group (IDG) that provides patient services during the week. The 1099 Hospice MSW Social Worker will complete regular visits for patients assigned to them ( averaging 25-30 visits a month ). They will also be expected to complete Admission Assessments in your service area on an as needed basis as well as applicable, discipline specific paperwork as needed. The schedule will be managed by the admin team but does allow for flexibility.

Our patients live where ever they call home - their living room, an assisted living facility or even a nursing home - we go to them. You will work together with our team to make sure that each individual receives the best, most personalized end-of-life care possible.

1099 Hospice MSW Social Worker Requirements and Responsibilities:

• MSW from a school of social work accredited by the Council on Social Work Education.
• Licensure and/or Certification as required by the state, or eligible for licensure by state. Preferred if not required by state.
• Hospice or home health experience preferred.
• A reliable source of transportation that is insured.
• Ability or willingness to use/learn technology.
• Hospice EMR software experience preferred.
• Ability to pass a background check.

1099 Hospice MSW Social Worker Job Details:

• Regular hours are Monday through Friday, 8am - 5pm
• 25-30 patient visits per month, on average
• 1099 employee with yearly total of $29,900 or $1,150/pay check
• Mileage reimbursement at the federal rate
• East Valley area / East of SR51

Schedule: Day Shift / Monday to Friday / On call

Work Location: On the road
