Your Key Responsibilities

• Performing thorough patient evaluations, ordering necessary diagnostics, and developing personalized care strategies
• Diagnosing and managing acute, episodic, or chronic conditions in concert with physicians or independently (per your credentialing)
• Instructing patients and families on preventive care, health maintenance, and effective disease management
• Prescribing necessary medications and treatments in accordance with your practice guidelines
• Engaging in patient follow-up and comprehensive chronic care management
• Coordinating with interdisciplinary teams to deliver cohesive and comprehensive care
• Meticulously documenting patient information and care plans using electronic medical records (EMR)

Essential Education Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Successful graduation from an accredited Nurse Practitioner program

Necessary Certifications

• Active Nurse Practitioner licensure in the state of Arizona
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or an equivalent specialty
