Job Title: Home Health Occupational Therapist (OT)
Job Summary
The Home Health Occupational Therapist evaluates and treats patients in their homes to improve independence, safety, and functional performance in daily living activities. The Occupational Therapist develops individualized treatment plans to help patients regain, develop, or maintain skills needed for self-care, mobility, and participation in meaningful activities following illness, injury, or disability.
Key Responsibilities
Perform initial and ongoing occupational therapy evaluations in the patient’s home environment.
Develop individualized treatment plans based on physician orders and patient needs.
Provide therapeutic interventions to improve activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
Assess home safety and recommend adaptive equipment, environmental modifications, and fall-prevention strategies.
Train patients and caregivers on therapeutic exercises, energy conservation, and safe functional mobility.
Monitor patient progress and adjust treatment plans as appropriate.
Document evaluations, treatments, progress notes, and discharge summaries in compliance with agency, state, and federal regulations.
Collaborate with physicians, nurses, physical therapists, speech therapists, and other interdisciplinary team members.
Participate in care conferences and communicate patient status updates.
Ensure compliance with Medicare, Medicaid, and home health regulatory standards.
Maintain patient confidentiality according to HIPAA guidelines.
Required Qualifications
Graduate of an accredited Occupational Therapy program.
Current, active Occupational Therapist license in the state of practice.
Current CPR/BLS certification.
Valid driver’s license, reliable transportation, and auto insurance.
Ability to travel to patient homes within assigned service area.
Preferred Qualifications
Minimum of one (1) year of occupational therapy experience.
Prior home health experience preferred.
Knowledge of OASIS documentation and home health regulations.
Experience working with geriatric and medically complex patients.
Skills & Competencies
Strong clinical assessment and treatment planning skills.
Excellent communication and patient education abilities.
Time management and independent decision-making skills.
Ability to work autonomously in a home-based setting.
Compassionate, patient-centered approach to care.
Work Environment
Services provided in patient homes across varied environments.
Frequent travel required.
Moderate physical activity including lifting, transferring, bending, and assisting patients.
Physical Requirements
Ability to lift and assist patients (up to 50 lbs with assistance as needed).
Ability to stand, walk, bend, kneel, and reach for extended periods.
Visual and auditory ability sufficient for patient assessment and treatment.
