As an RN Clinical Manager, you will coordinate and manage patient care, ensuring that services are delivered appropriately. You will also supervise clinical personnel to maintain high-quality care and improve patient outcomes.

Overview:

• Ensure availability of Clinical Manager during all operating hours
• Develop and update individualized plan of care in collaboration with physicians
• Review patient information and assign appropriate home care personnel
• Confer with Attending Physician to determine eligibility for services
• Review and evaluate each case, providing guidance to clinicians
• Assist in establishing therapeutic goals and developing a plan of care
• Participate in case conference meetings and record reviews
• Assist in screening, interviewing, and orienting new personnel
• Contribute to in-service and continuing education programs
• Comply with professional standards and participate in quality assessment programs
• Perform other duties as delegated by the Director of Nursing and/or Branch Manager

Job Requirements:

• Registered Nurse with current licensure to practice in the State
• Previous experience in a home care setting required
• 2 years of management or supervisory experience preferred
• Ability to work within an interdisciplinary setting
• Excellent observation, nursing judgment, and communication skills
• Valid driver's license and insured automobile in good working order
• Current CPR certification
