An established outpatient rehabilitation clinic in Flagstaff, AZ is seeking a Full-time or Per Diem Physical Therapist to join its team on a flexible, as-needed basis. Compensation & Incentives

• Estimated Annual Earnings: $85,000 – $100,000 (dependent on availability and experience)
• Monthly Bonus Potential: $500 – $2,000 (based on RVU productivity)
• Sign-On / Relocation Incentive: $15,000 – $20,000 for full-time hires

Key Responsibilities

• Perform comprehensive initial evaluations and periodic reassessments
• Develop individualized treatment plans using evidence-based techniques
• Provide hands-on care, exercise instruction, and patient education
• Maintain accurate and timely documentation in the EMR system
• Collaborate with physicians and interdisciplinary care teams
• Educate patients on home exercise programs and strategies for long-term management
• Uphold professional, ethical, and clinical compliance standards

Candidate Qualifications

• Graduate of a CAPTE-accredited U.S. Physical Therapy program
• Master’s degree required; Doctor of Physical Therapy (DPT) preferred
• Current Arizona Physical Therapist license (or license-eligible)
• Strong communication, organizational, and clinical reasoning skills
• Ability to independently manage a variable caseload
• Prior outpatient or orthopedic experience preferred but not required
