The Connections Care Coordinator is responsible for maintaining and monitoring the memory care (Connections) holistic wellness program, including but not restricted to: assisting the Director of Engagement with input into resident assessments, leading neuroplasticity programming (Connect-Move-Learn cycles),, muscular strength, aerobic and flexibility training, wellness promotion, social-recreational activities and outings and volunteer program development and community resource coordination.

Highlights of the Connections Care Coordinator Position :
- High school diploma or equivalent
- Minimum 1 year experience activities or recreation service experience
- Certified Dementia Practitioner or eligible for certification.
- Willingness to work in a hands-on role with residents and staff.
- Must be able to drive a company vehicle (town car and 14 passenger van)
- Must possess a current driver's license and ability to meet approved driver requirements
