Key Responsibilities:

• Develop and implement individualized care plans for patients in their homes.

• Perform assessments and provide education and support to patients and families.

• Coordinate home health services to ensure high-quality, continuous care.

• Collaborate with a dedicated team of healthcare professionals to achieve optimal outcomes.

Qualifications:

• Active RN license in the state of Arizona

• Valid driver’s license and reliable transportation

• Current CPR/BLS certification (or willingness to obtain)

• TB test and fingerprint clearance (we can assist with this process)

• Prior home health experience required

• Bilingual (Spanish) is a plus

• Graduate of an accredited RN program
