Registered Nurse (RN) - Hospice Patient Care Manager

Key Responsibilities:

• Oversee the day-to-day operations of the hospice care team.

• Coordinate and supervise patient care to ensure high-quality, compliant services.

• Provide clinical guidance and support to field staff.

• Monitor patient progress and collaborate with interdisciplinary teams to update care plans.

• Ensure compliance with all state and federal hospice regulations.

• Act as a liaison between patients, families, physicians, and caregivers.

Qualifications:

• Active and unrestricted RN license in Arizona (or compact state license).

• 1–2 years of nursing experience , with hospice or palliative care strongly preferred.

• Strong leadership, organizational, and communication skills.

• Passion for compassionate end-of-life care.

• Valid driver’s license and reliable transportation.
