And we do it all with heart, each and every day. **Position Summary** The **Care Management Associate** initiates contact with members within three days of discharge from a hospital or facility. This person ensures outreach is focused on discharge planning and scheduling follow-up visits with the member's assigned Primary Care Physician ( _PCP_ ). **In this role, you will:** + Apply critical thinking and clinical judgment to support the case management process. + Collaborate with internal teams and external providers to facilitate appropriate healthcare outcomes. + Use care management tools and resources to educate and support members. + Promote and support the quality administration of healthcare services. + Ensure that all actions align with organizational standards and regulatory requirements. **Required Qualifications** + 1-2 years of experience as a medical assistant, office assistant, or adjacent role. + 1-2 years of experience in a call center environment. + Must reside in Arizona. **Preferred Qualifications** + Experience working in managed care. + Effective communication, telephonic, and organization skills. + Familiarity with basic medical terminology and concepts used in care management. + Strong customer service skills to coordinate service delivery, including attention to customers, sensitivity to issues, proactive identification, and resolution of issues to promote positive outcomes for members. + Computer literacy to navigate through internal/external computer systems, including Microsoft Excel and Microsoft Word. **Education** + High school diploma or GED. **Anticipated Weekly Hours** 40 **Time Type** Full time **Pay Range** The typical pay range for this role is: $18.50 - $35.29 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.
