Orion Allied Healthcare is seeking experienced Case Manager Registered Nurses (RNs)for a travel assignment in Tuba City, AZ. This is a great opportunity to provide critical care in a supportive environment while enjoying the lifestyle of Arizona.

- Hours: 40 hours per week

Responsibilities
- They advocate for patient safety and quality of care, ensuring patient needs are met and that patients and families understand their treatment options.
- They act as the central point of contact, coordinating care among physicians, specialists, home health nurses, and other healthcare professionals to ensure continuity of care.
- They develop and adjust care plans based on patient needs, which can include scheduling follow-up appointments, therapies, and home care services.
- They educate patients and families about their condition and how to manage it, while also connecting them with community services, financial assistance, and other resources.
- They monitor patient progress, review medical test results, and maintain detailed records of medical updates and assessments.
Requirements
- Minimum of 1 year of recent Case Manager RN experience
- Active RN license
- Strong critical care skills and ability to perform in high-acuity settings
