• Conduct in-home health assessments and create personalized care plans.

• Administer medications, treatments, and wound care as prescribed.

• Educate patients and families on managing conditions and promoting independence.

• Monitor patient progress and adjust care plans as needed.

• Supervise LPNs and Home Health Aides to ensure high-quality care.

• Participate in interdisciplinary meetings to coordinate comprehensive patient care.

• Active RN license in Arizona.

• Minimum of one year of RN experience ( home health, hospice, or critical care preferred ).

• CPR certification and a valid driver’s license with reliable transportation.

• Strong communication, organizational, and interpersonal skills .
