Housing Solutions Case Manager

Pay Rate: $22.00-$22.50/hour
Schedule: Tuesday - Saturday: 8:00 am - 4:30pm

Job Summary: The Housing Solutions Case Manager/Housing Navigator is responsible for assisting Mercy House clients in obtaining and sustaining permanent housing by utilizing the Housing First Model. The Housing Solutions Case Manager is responsible for providing case management services, housing advocacy and navigation, and supportive services to clients enrolled in the Adult Re-entry Program. This position reports directly to the Housing Solutions Supervisor.

Essential Duties and Responsibilities:

  • Provide comprehensive Case Management services including intakes, housing stabilization plans, goals planning, linking, and monitoring
  • Provide tenant education, advocacy, and referrals to various services as needed
  • Provide housing search and placement services
  • Develop an individual case management plan, including financial planning, and housing goals with recipient individuals and/or families
  • Meet regularly with clients to track progress in the program and ensure they are in compliance with grant requirements
  • Maintain case files up to date and ensure that all necessary client information is entered into HMIS in a timely manner
  • Coordinate home visits with clients as necessary
  • Conduct Habitability Inspections in coordination with the Housing Solutions Agents as needed
  • Assist clients housing related barriers
  • Make referrals to Chrysalis for job placement services
  • Assist clients to build upon their strengths and recognize their needs in a respectful manner
  • Be knowledgeable of community resources and link clients to such resources as needed
  • Follow all case management policies within the agency
  • Maintain a case load as assigned of up to 25 clients
  • Participate in Coordinated Entry System meetings weekly, following all CES policies and procedures to request client matches and submit updates as needed
  • Assist clients in searching for housing leads and facilitate relationships between landlords and clients
  • Provide support and mediate landlord/resident issues as necessary to help client sustain their housing
  • Complete re-certification of assistance as necessary and gather required documentation from clients

Team/Staff Relations

  • Work as a Mercy House team member with all staff, interns and/or volunteers in order to implement the mission and core values of the agency
  • Attend staff meetings, supervision meetings, community meetings, and training workshops as needed
  • Attend monthly meetings with the Housing Solutions team to discuss client progress, needs, and share resources
  • Accept the responsibility for aiding the development of a positive team relationship with other case managers and upper management

Administration

  • Keep accurate and updated documentation from all clients, including but not limited to stabilization plans, case notes, etc.
  • Receive and follow-up with referrals for the program from partner agencies including the Orange County Probation Department and local shelters
  • Assist with program activities when necessary
  • Develop new partnerships with other agencies in order to enhance current services
  • Assist with all data collection systems
  • Ensure the confidentiality and safety of case files and client information
  • Ensure documents submitted on behalf of the client are valid and submitted on a timely basis
  • Perform ad hoc projects as appointed by Housing Solutions Supervisor and/or Manage

Requirements:

Education and Experience

  • Bachelor's Degree in Human Services, Social Work, or related field is highly preferred
  • At least 2 years of direct life experience working with formerly incarcerated homeless individuals, chronically homeless, low income, and have a working knowledge of mental health and addiction issues is of great value
  • Must be proficient in Microsoft Office programs (Outlook, Word, and Excel)
  • Must possess basic typing, email, and internet skills

General

  • A valid CA driver's license, proof of insurance and a reliable vehicle is required. Comfortable with utilizing your own vehicle to conduct home visits, visiting clients at shelters, meeting clients out in the community etc.
  • Dependability, responsibility, and the ability to communicate effectively are required skills
  • Desired skills include organizational skills, strong written and verbal communication skills, critical thinking skills, attention to detail, and client-oriented
  • Must have the ability to work in a fast-paced environment and have the ability to recognize and maintain confidentiality of sensitive information
  • Must work effectively with a diverse population and demonstrate ethical behavior
  • Must be able to plan, organize and prioritize duties; speak effectively before small groups; perform crisis intervention as needed; clearly communicate information and instructions verbally and in written form; maintain a positive, professional, and safe environment while on duty; and establish and maintain effective working relationships with clients, landlords, and other service providers
  • Fluency in Spanish is of significant value

Physical Requirements

  • Ability to lift, bend, push, and pull up to 50 pounds
  • Prolonged periods sitting at a desk and working on a computer
