Your Role:Develop Individualized Care Plans to support patient recovery and mobility goals.
Provide Patient & Family Education to encourage continued progress and independence.
Collaborate with the Healthcare Team to coordinate quality, patient-centered care.
Monitor and Document Patient Progress to ensure effective and goal-oriented outcomes.
Licensure: Active Physical Therapy license in the State of Arizona.
Skills: Strong communication and interpersonal abilities to foster trust and motivation.
Approach: Patient-centered, compassionate, and adaptable care mindset.
Flexibility: Willingness to travel locally and make a meaningful impact every day.
