*8 Sessions/week minimum* About Us: Places You’ll Go Therapy has a vision to become a leading provider of healthcare services, specializing in home health. Currently, we are primarily servicing the pediatric population between the ages of 3 and 17 years old. However, with continued growth, we are looking to expand our services to treat patients throughout the lifespan. We are committed to delivering high-quality care in the comfort of our patients' homes. Our team of dedicated professionals strives to make a positive impact on the lives of our patients and their families.Position Overview: We are currently seeking a skilled and compassionate Physical Therapist to join our team. The chosen candidate will work in a home health setting primarily with pediatric patients, providing personalized and effective physical therapy services. Responsibilities:
- Conduct thorough assessments of pediatric patients' physical therapy needs.
- Develop and implement individualized treatment plans to address specific goals.
- Collaborate with parents, caregivers, and other healthcare professionals to ensure comprehensive care.
- Provide ongoing evaluation and modify treatment plans as needed.
- Maintain accurate and up-to-date patient records.
- Provide supervision of Physical Therapist Assistants.
Qualifications:
- Active Arizona State Physical Therapist License.
- Current CPR certification.
- Article 9 Certification
- Valid driver's license and reliable transportation.
- Ability to work independently and as part of a collaborative team.
- Article 9 Certification
