Family Nurse Practitioner - Concierge Functional Primary Care (Part-Time)

Lifespann Primary Care & Esthetics is a boutique medical clinic dedicated to longevity, personalized care, and whole-person wellness. Our practice combines traditional primary care with functional and regenerative medicine to address the root causes of health concerns.

We operate in a concierge-style environment where time, attention, and strong patient relationships are central to the care we provide. Our goal is to deliver thoughtful, personalized medicine that focuses on prevention, lifestyle optimization, and long-term wellness.

The Family Nurse Practitioner will provide comprehensive primary care services while integrating functional and preventive approaches to patient care. This role includes conducting in-depth patient evaluations, reviewing laboratory results, developing individualized treatment plans, and guiding patients through lifestyle and wellness strategies.

The ideal candidate is a compassionate provider who values quality over volume, enjoys spending meaningful time with patients, and is passionate about preventive and root-cause medicine.

Key Responsibilities

• Provide comprehensive primary care assessment, diagnosis, and treatment
• Conduct extended patient consultations (30-60 minutes) to evaluate overall health and wellness
• Review and interpret laboratory results and diagnostic testing
• Develop personalized treatment plans focused on prevention and root-cause medicine
• Prescribe and manage therapies, including hormone replacement therapy and integrative treatments when appropriate
• Provide patient education on nutrition, lifestyle, metabolic health, and disease prevention
• Maintain accurate electronic medical record documentation
• Collaborate with the clinical team to deliver a high-quality concierge patient experience

Work Schedule

• Part-time position with potential to expand.

Clinic Hours

• Thursday: 8:00 AM - 6:00 PM
• Friday: 8:00 AM - 12:00 PM

Appointment Model:

• 30-60 minute visits
• Low patient volume
• High-touch patient care

Responsibilities

• Provide comprehensive primary care services, including health assessments, diagnosis, and treatment of acute and chronic conditions
• Conduct extended patient consultations (30-60 minutes) to evaluate medical history, lifestyle, nutrition, and overall wellness
• Review and interpret advanced laboratory testing and explain results in a clear, patient-centered manner
• Develop personalized treatment plans that may include functional medicine approaches, lifestyle modification, nutrition guidance, and preventive care strategies
• Prescribe and manage therapies such as hormone replacement therapy (HRT), peptide therapy, and other integrative medical treatments when appropriate
• Administer or oversee IV nutrient therapy and wellness treatments in accordance with clinical protocols
• Educate patients on gut health, inflammation management, metabolic health, and long-term disease prevention
• Maintain accurate and timely electronic medical records (EMR) documentation
• Collaborate with the clinical team to deliver a high-touch, concierge-level patient experience
• Support clinic growth through patient education, wellness program participation, and service development
• Ensure compliance with state regulations, clinical guidelines, and best practices in primary and integrative care

Qualifications

• Active Arizona Family Nurse Practitioner license
• Board Certified FNP (AANP or ANCC)
• Minimum 2-3 years of experience in primary care
• Strong clinical judgment and patient assessment skills
• Excellent communication and bedside manner
• Passion for preventive and functional medicine
• Comfortable working in a concierge-style environment
