Brighter Days Home Health is seeking an experienced and dependable Registered Nurse (RN) to join our growing home health team on a PRN basis.

Responsibilities:

• Perform skilled nursing visits in accordance with physician orders and the plan of care.
• Complete patient assessments, admissions, recertifications, resumptions of care, and discharges as assigned.
• Administer medications, wound care, and treatments.
• Monitor patient condition and communicate changes appropriately.
• Educate patients and caregivers regarding medications, disease processes, safety, and ongoing care needs.
• Accurately complete documentation in compliance with agency policies and Medicare Conditions of Participation (CoPs).
• Coordinate care with physicians, therapists, case managers, and interdisciplinary team members.
• Participate in case conferences and care coordination activities.

Qualifications:

• Current Arizona RN license in good standing.
• Graduate of an accredited nursing program.
• Minimum of 1 year nursing experience required.
• Home health experience preferred.
• Oasis documentation experience preferred.
• Current CPR certification.
• Reliable transportation, valid driver’s license, and current auto insurance.
• Strong clinical judgment, communication, and organizational skills.
• Ability to work independently and manage time effectively.

Compensation:

• Competitive per-visit rates based on visit type and experience.
• Additional compensation may be available for admissions, high-acuity visits, extended travel areas, and weekends.
