POSITION TITLE:

Healthcare Outreach & Referral Specialist

PURPOSE OF ROLE:

To promote Violet Residential Service Agency (VRSA) by creating strong, lasting relationships with hospitals, skilled nursing facilities, hospice agencies, and community partners. The goal is to generate private pay and Long-Term Care (LTC) referrals and help VRSA grow its client base across Arizona.

POSITION TYPE:

Per Diem (PRN) – work as needed for outreach visits, calls, and networking events.
This role is hourly and commission-based.

COMPENSATION STRUCTURE

Base Pay:

• $18.00/hour for verified outreach activities (facility visits, calls, meetings).

Commission:

• $150.00 for every new client referral that converts (client must sign a service agreement and begin care).

Mileage Reimbursement:

• $0.70 per mile for approved outreach travel.

SCHEDULE EXPECTATION

• Flexible PRN schedule , typically Monday, Wednesday, and Friday.
• Conduct outreach to hospitals, rehab centers, senior living communities, LTC agents, and community organizations within assigned areas.
• Must be available to start immediately.

PRIMARY RESPONSIBILITIES

• Represent VRSA with professionalism, enthusiasm, and reliability.
• Build new referral relationships through networking and consistent follow-ups.
• Visit and present VRSA’s services to case managers, discharge planners, and social workers.
• Distribute flyers, brochures, and business cards to potential partners.
• Track all contacts and outreach activities in the referral log.
• Provide weekly updates to management every Friday.

QUALIFICATIONS (REQUIRED)

• Minimum 1 year of experience in home care, hospice, healthcare marketing, or community outreach.
• Strong communication and networking skills.
• Highly reliable, organized, and self-motivated “go-getter.”
• Proven ability to connect and build relationships in healthcare or community settings.
• Strong problem solving and critical thinking skills
• Deadline and detail-oriented
• Valid driver’s license and reliable transportation.
• Willing and ready to start immediately.

• Business cards, flyers, and brochures.
• Access to referral tracking tools.
• Ongoing guidance, mentorship, and management support.
