Responsibilities
- Collaborate with nursing staff and providers in the Surgery Outpatient Clinic to coordinate patient care and ensure smooth care transitions
- Lead case management activities, including patient assessment, care planning, resource coordination, and barrier identification to support safe discharge
- Facilitate discharge planning and coordinate post-acute care services, equipment, or home health as needed
- Monitor patient progress, document care activities, and communicate updates to the multidisciplinary care team
- May float to other departments as needed, including taking a patient load or performing task-oriented duties such as answering call lights and coordinating administrative tasks
- Ensure compliance with relevant policies, regulations, and patient safety standards

Requirements
- Active Registered Nurse license
- Three (3) years of case management experience
- Ability to work Monday through Friday, 8:00 a.m.–5:00 p.m., with no weekend or major holiday shifts
- Willingness to float to other departments as needed during the assignment
- Availability to work a minimum of 36 hours per week
