Desert Ridge Family Physicians is looking for a Chronic Care Manager / Supervisor to lead our Chronic Care Management (CCM) team. In this dual-role, you’ll provide hands-on care to patients with chronic conditions and guide a team to deliver exceptional, coordinated care.

Key Responsibilities

Chronic Care Management

• Collaborate with physicians, patients, families, and healthcare staff to coordinate chronic care programs.
• Develop and implement individualized care plans aligned with patients’ medical needs and personal goals.
• Educate patients on preventive care, medications, and community resources.
• Conduct medication reconciliation and follow-up assessments to monitor ongoing progress.
• Reduce ER utilization and hospital readmissions through proactive care coordination.

CCM Supervisor Responsibilities

• Monitor employee performance and provide feedback and coaching to build skills and confidence.
• Train and onboard new employees to ensure consistency and quality of care.
• Manage staff schedules, approve time off requests, and oversee workflow and productivity.
• Implement process improvements and ensure regulatory compliance.
• Serve as the bridge between leadership and front-line staff, reporting team metrics and outcomes.
• Facilitate team meetings to ensure alignment and collaboration.

Who You Are

• Experienced Chronic Care Manager or care coordinator.
• Minimum 3 years as a Medical Assistant (certification preferred).
• Proven leadership or supervisory experience required.
• Strong organizational, problem-solving, and communication skills.
• Compassionate, patient-centered, and eager to improve care outcomes.
• Experience with Athena EHR or similar systems is preferred.
